Lower face, below nose, lips, chin. Lips were abraded. Between nose and upper lip a 1/8" semi-lunar abrasion, a 1/4" abrasion on lateral lower lip, a 1/8" abrasion on midline of lips. A 3/16" laceration on left upper lip. Lips were abraded with superficial lacerations.
Upper face, forehead, nose and eyes Multiple punctate scratches over bridge of nose. Abraded left ala [nasi?]. Left forehead 1 5/8 x 1 1/8" abraded laceration with a 1/2" abrasion on its superior margin. Two contusions on right forehead, 1/4 x 1/2" and 1/2 x 1/2". A 3/16" abrasion on superior aspect of bridge of nose. Two abrasions above left eyebrow, 1" and 3/4". 1/2 x 1/2" contusion on right eyelid. 1/8" abrasion adjacent to medial aspect of left eye. 1/8" abrasion adjacent to lateral aspect of left eyebrow. 3/4" old scar on right forehead. 1/4 inch old scar adjacent to the bridge of the nose. 1/2" contusion in right medial periorbital region. 2" scratch below right eye. Above and below left eyebrow a bell shaped patterned abrasion with a base of 3/4" and 1 1/4" between base and dome. 1/4" laceration adjacent to superior medial margin.
Exterior cheeks, mandibles. Left side of cheek contused and edematous with overlying 1 1/2" contusion. Linear scratches on left mandible along with 3 x 3 3/4" area of abrasion. 4 x 3/4" contusion on right zygomatic region with overlying punctate abrasions and a 3/16 by 1/2" abrasion. Two 3/16" circular abrasions on left zygomatic region and 1/4 by 1/8 abrasion below. Multiple scratches over right mandible. Bell-shaped abrasion on right mandible with central area of pallor and abrasion. Area measuring 5 1/2 x 5" on left side of face including left ear had multiple gouging type irregular cutting wounds from 1/8" to 1 3/4" with many terminating in the oral cavity.
Interior of mouth Contused mucosal surfaces of lips and slightly edematous, with multiple superficial lacerations. Multiple bite marks on lateral margins and tip of tongue. Multiple superficial bitemarks on inside of cheeks. Mucosal surface of lower lip showed a 5/16" laceration. Frenulum contused and surrounded by a 1/2" contusion. Multple contusions, lacerations, and hemorrhage on mucosal surfaces. Gums hemorrhagic.
Right ear Contused anterior and posterior surfaces with overlaying linear scratches. Helix abraded. 1 3/4 x 3/4" ovoid contusion with edema behind ear. Multiple semilunar scratches behind ear. Abraded and contused, multiple linear abrasions on inferior aspect, 1/2" to 1 1/4". Confluent contusions and abrasions.
Left ear No injuries, contusions or abrasions mentioned. Contused, five linear abrasions behind it, 2 were 1/2", 2 were 7/16", and one 3/4". In front of left ear, five haphazardly oriented abrasions from 5/16" to 1 3/4". Entire ear contused with overlying finely linear abrasions.
Scalp Right frontal scalp, 2 1/2 x 1/2" area of edema and ovoid contusion with multiple overlying superficial lacerations and a 1/8" depressed abraded laceration. 2 3/4 x 1 1/2" ovoid contusion on right parietal scalp. 3/4 x 1/8" dove-tail laceration on left parietal scalp with a patterned contusion in the form of an upside down "L" at its inferior margin with the dimensions of 1/2" vertical and 1/4" horizontal. 1 1/8 x 1/4" abraded contusion on left temporal scalp. Multifocal extensive subgaleal contusions and edema. 1 1/4" laceration on left parietal scalp. Reflection of scalp showed hemorrhage in soft tissues underlying the laceration. Multifocal subgaleal contusion with edema. Multiple superficial cuts and abrasions to the left parietal scalp. Left occipital scalp edematous with contusion and overlying abrasions measuring 4" in greatest dimension. Reflection of scalp showed multifocal subgaleal contusions.
Skull 1 1/4" semi-lunar fracture on right posterior frontal skull bone. Immediately below a 2" similar semi-lunar fracture. Below and lateral to this a 1" fracture. Extending over the temporal scalp were two contiguous fractures; the superior one measured 1 3/4" and the intersecting semi-lunar inferior one measured 1" in length. Three semilunar fractures one which became contiguous with the second, overall dimension 2". Inferior to this was a similiar semi-lunar fracture measuring 1 3/4". A 3/4" fracture involving the right anterior cranial fossae. Multiple fractures, base of skull. Left posterior cranial fossae 3 1/2" fracture with radiating fractures. Left posterior medial cranial fossa 1/4" ovoid punched out fracture. Left middle cranial fossa 1" fracture. 3 1/2" fracture at base of skull with multiple extension fractures terminating at the foramen magnum.
Brain Edematous with subarachnoid hemorrhage involving right cerebellar hemisphere. Fracture contusions involving right posterior cerebellar hemisphere. Multifocal subarachnoid hemorrhage involving left posterior and cerebellar hemispheres. Associated fracture contusions. Multifocal subarachnoid hemorrhages to left posterior cerebral hemisphere.
Neck Right side, scapula region, contusion area with overlying 1 1/4 x 1/2" abrasion. Medial to this, extending to left side of neck, 1 x 3/4" contusion. Few scattered abrasions, left side. Extravasated hemorrage in posterior neck muscles.
Chest, shoulders, abdomen Right shoulder three scattered contusions 1/4 to 1/2" with an adjacent area of focal red-purple contusion. Below this contusion an area, 2 1/4 x 1/4", were multiple linear, diagonally oriented abrasions surrounded by contusions. These abrasions were interspaced by 1/8 to 1/4". Lower right chest were two 3/4" contusions. Below this contusion were two parallel oriented abrasions 3/4" each and interspaced by 1/8". Over left side of abdomen, a group of linear abrasions interspaced 1/16 x 1/8". Old hypopigmented scar, front of chest. Multiple scattered abrasions, front of chest.
Genitals No injuries. Skin of penis, scrotal sac and testes missing with gaping defect of 2 3/4 x 1 1/2". Shaft of penis present. Gaping defect surrounded by multiple and extensive irregular punctate gouging type injuries, 1/8 to 3/4" with depth of 1/4 to 1/2". Some of these wounds showed hemorrhage in underlying soft tissue, others did not. In between the thighs were multiple areas of yellow abrasions with skin slippage. Medial aspect of left thigh showed a yellow abrasion. Microscopic exam of penis revealed bacterial colonies. Midshaft of penis including glans diffusely red-purple with overlying very fine superficial scratches with clear band of demarcation showing proximal portion uninvolved. Blood vessel engorgement of head and shaft under microscopic analysis.
Anus, rectum and buttocks Over right buttocks were multiple linear scratches measuring from 1/2 to 3/8". On inferior aspect of left buttocks, punctate linear scratches. Anus dilated, no external evidence of injury. Mud and debris was present in the anal orifice. Mucosal surfaces of rectum were slightly hyperemic and showed no evidence of injury. Microscopic examination revealed no hemorrhage. Left buttocks, 1/2 x 1/4" contusion and 1 3/4" linear abrasion and five superficial cutting wounds measuring 1/2 to 2 1/8". Right buttocks, two faint contusions, each 1/2 x 1/2". Also right buttocks, multiple linear superficial interrupted cuts 3/16 to 1/2", intespaced by 1/8". Scattered linear abrasions about the anus. Anal and rectal mucosae were hyperemic and injected. Anus dilated. Abundant feces found about the anus. No semen or spermatozoa found or blood found in swabs. No hemorrhage under microscopic analysis. Anus dilated. Anal and rectal mucosa showed mild hyperemia, no evidence of injury. No abrasions, contusions or lacerations. No hemorrhage or spermatozoa under microscopic analysis.
Lower extremities A 1/4" ovoid contusion over left knee. A 1 x 1 3/8" reddish contusion, right anterior thigh. Below this, patterned contusion consisting of two ovoid red-purple contusion, the superior one 3/4 x 7/16" and the inferior one 3/4 x 1 1/8". Extending from both of these were linear contusions, the superior one 5 3/16", the inferior one 4 1/2". The interspace between these was 3/4". A 1/4" abrasion on anterior surface of right leg. Over posterior right thigh and leg were multiple scattered abrasions and contusions. (Listed under injuries to left leg: A 1/4" abrasion on right knee. Below right knee three red contusions, 3/4, 1/4, and 1/2".) The posterior left leg and ankle showed confluent contusions. On left ankle, below binding abrasion, a 3/4 x 1/2" abrasion. On left ankle, a 3/4 x 1" abrasion. Above ligature binding site on right ankle a 2 1/2 x 2 3/4" contusion. A few old healed scars. Multiple scratches and contusions. Left thigh 1" yellow scratch. Also, left thigh, patterned grid-like impression with margins diagonally oriented, 3" lateral x 3 1/2" medial.
Binding abrasions Removing of binding showed abraded and contused furrows present on both right and left ankles and wrists. Lower: binding abrasions on both ankles with faint red-purple contusions above and below. Upper: contusions around binding abrasions on wrists. Lower: yellow-red abrasions on ankles. Upper: Yellow binding abrasion not surrounded by contusion was present on right wrist. Binding abrasion on left wrist with a faint red-purple contusion at superior margin. Lower: Abrasions and contusions involving the ankles. Abrasion yellow ten with abraded margins. Upper: Abrasion surrounded by red contusions on wrists.
Upper extremities 1" contusion back of forearm. 1/8" abrasion on left antecubital fossa. Below this linear abrasions measuring from 1/16 to 1/2". 1" cut on right thenar eminence. 1/8" superficial laceration on left thenar eminence. 3/4" scratch back of left hand. 1/16" abrasion on anterior surface of left thumb. Scattered abrasions right arm and forearm. Multiple scattered abrasions anteroposterior surfaces of left arm. Back of hands and thenar eminences, multiple scattered contusions.
Back Upper right back, two diagonally oriented interrupted abrasions, each measuring 4 1/2", interspaced each 1/2". Below this abrasion on left side was a 4 x 2" contusion. Below this contusion were two linear oriented abrasions which were interspaced by 1/8". Multiple scattered abrasions, upper back. Two contusions upper left back 2 x 1 1/2" and 2 x 1".
Internal, respiratory Sphenoid sinus contained 2 ml of bloody fluid. Lungs were edematous and extruded abundant amounts of frothy material. Upper and lower airways contained bloody frothy material mixed with brown vomitus. No abnormal collections of fluid were present in any of the body cavities. Upper and lower airways contained slight amount of vomitus. Mucosal respiratory surfaces were hyperemic. Pulmonary parenchyma exuded slight amounts of blood and frothy fluid. Pulmonary edema with congestion with bloody frothy fluid in airways. Slight amount of vomitus. Bloody watery fluid in sphenoid sinus.
Internal, alimentary Stomach lumen contained 2 ounces of brown fluid. Small and large bowels unremarkable. Stomach lumen contained 1 ounce red-tan fluid with piece of chewing gum. Small and large bowels unremarkable. Stomach. Lumen contained 2 ounces of partially digested fluid and remnants of green vegetable-like material. Small and large bowels unremarkable.
